Your Patient ID Card

As someone with a CRT-P or CRT-D, you may receive an identification card to carry in your wallet. This tells healthcare professionals and other key personnel vital details about your implanted device in case you need emergency care.

When you travel and pass through metal detectors, your device may trigger the alarm. If this happens, present your implant ID card to security personnel.

If you do not receive your permanent, plastic ID card within 90 days of your procedure, or if you need a replacement card, contact St. Jude Medical to request a card today.

Make it a routine to tell your other doctors, dentists and healthcare professionals that you have a CRT-P or CRT-D before they treat you. You should also wear a visible medical identification bracelet that identifies the kind of device you have. Look on the Web, check with your pharmacist or healthcare providers for information on available products.
