Your Patient ID Card

    Your St. Jude Medical ID card identifies you as a pacemaker, implantable cardioverter defibrillator (ICD) or heart valve recipient. The card includes information about your medical device and emergency physician contact information.

    Card Maintenance Tips

    Your card will be most useful if you follow these tips:

    • Contact us if you do not receive your permanent, plastic ID card within 90 days after surgery.
    • Always carry your card with you.
    • Keep St. Jude Medical updated when your contact information or physician or cardiologist change. You can update your records online with our device registration form or you can contact us.

    Note: Online updates are available only to device recipients located in the United States, Canada and Australia. If you live in a country other than these three, please contact the St. Jude Medical distributor in your area. For contact information, go to Locations.
